OVERVIEW OF PRESCRIPTION PLAN The Town provides prescription coverage through RxBenefits. This is included in the Town's medical insurance. There is no additional per paycheck cost for this benefit. This prescription plan covers generic medications at 100%. For non-generic medications there is an additional out-of-pocket expense separate from the medical out-of-pocket maximum. This amount is $1,500 individual coverage or $3,000 for family coverage. Your RxBenefits are accepted at most pharmacies nationwide, but if you would like to receive full advantage of the benefits available to you, choose one of these PreferredChoice Network Participating Retail Pharmacies. GETTING THE MOST FROM YOUR PRESCRIPTION PLAN To get the most from your prescription plan, the Town encourages you to review the following: • Prescriptions for Generic Medications – If a generic is available, the plan will only pay for the generic drug. If you choose to buy the Brand name drug in this situation, you will be required to pay the Brand co-pay plus the difference in cost between the Generic and Brand name drug. • Requesting a 90-Day Supply for Maintenance Medications – If you take a main- tenance medication for an ongoing condition, you are encouraged to sign up for a 90- day supply and save money. You can receive a 3 month supply for the cost of 2 months. • Utilizing Mail Order – A 90-day prescription can be filled at your local CVS or Target OR you can have your physician submit your prescription directly to CVS/Caremark's Mail Order Service using the FastStart program. You will need to have your member ID # and your ID card available and you will need to register at or contact RxBene- fits at 1-800-334-8134 for assistance. • Diabetic Services - The Town has partnered with CCS Medical to offer LivingConnected. This program offers services to help educate and help monitor Diabetes. A certified diabetes educator will make outreach to the employee to inform the employee of all the FREE ben- efits of LivingConnected. Answer any questions and help provide any coaching they would like to receive. The employee will also receive a new smart glucose meter, after their initial welcome call.
