Bariatric Surgery, Cardiac Care, Complex and Rare Cancers, Knee and Hip

• Care Management – Livongo is a FREE benefit to support you with your diabetes,
pre-diabetes, high blood pressure and other health goals through technology and

• Case Management - During times of unexpected medical expenses,
BCBSNC offers
Case Management to help you navigate the health care system.

• Diabetic Supplies – To ensure diabetics and pre-diabetics have the supplies
they need, visit the list of
BCBSNC Diabetic Supplies Covered. Many are free. Also
contact Livongo at 1-800-945-4355, use code BCNC2, and learn more
about Livongo's diabetes program.

• Durable Medical Equipment (DME) – Check out this listing of providers within a
25 mile radius of Town Hall. Edgepark's dedicated case manager can be contacted
at 844-564-1007 or cmsupport@edgepark.com. There are also instructions for
using a different provider.
• Maternity Program – The MyPregnancy Program supports expecting mothers.
Those who complete the program and submit required documentation are eligible
for a $250 reimbursement.
• NurseLine – The BCBS NurseLine is available 24 hours a day, 7 days a week, with
trained nurses who answer medical questions, offer support, and send videos and
brochures on health topics. Call 1-877-477-2424.
• Preventive Care Guide – The Preventive Care Guidelines are an excellent
source of information for ensuring you're receiving the preventive care
services you need!
• Tobacco Cessation Programs – Tobacco cessation programs are covered at 100%.
If you are charged for a program, you are eligible for reimbursement through Cary's
Health Reimbursement Arrangement program.
• Blue365 – Discounts are available to members of BCBSNC and their dependents
through Blue365. Discounts can include. apparel and footwear, fitness, hearing
and vision, nutrition, personal care, and travel. There is no cost to join. These
Blue365 wellness discounts can include apparel & footwear, fitness, hearing &
