OVERVIEW OF THE DENTAL PLAN
Cary provides dental insurance through Delta Dental. The dental plan covers free exams
and cleaning with no limit per plan year. The plan also covers other diagnostic and
preventative services.
For all services (except surgical extractions, general anesthesia and orthodontia), the plan
will pay up to a maximum of $2,000 per person per benefit year on all services. The plan will
cover up to $4,000 per person per lifetime on orthodontic services.
COST OF THE DENTAL PLAN
The cost of the dental insurance depends on who the employee chooses to cover. Below are
the current rates per paycheck:

*If you elect to cover a domestic partner, any premium (or rate) for the domestic
partner and/or domestic partner child(ren) will be paid post-tax and reflected as such on

CHOOSING A DENTIST
As a Delta Dental PPO or Premier member, you may see any dentist you like. However,
there are advantages to choosing a dentist who belongs to one of Delta Dental's two
dentist networks. Below is an example of basic benefits performed by PPO, Premier and
Out-of-Network Dentists.
