CONTRIBUTIONS
Cary contributes $3,500 to this account. The amount may change per year. You cannot
contribute to this account.
AVAILABLE FUNDS
Funds are automatically available on the first day of coverage. If you enroll in the HDHP,
Cary will contribute your $3500 Benefits My Way into an HSA instead.
TYPES OF ELIGIBLE EXPENSES
Eligible expenses span a variety of categories, including but not limited to, items used for
educational, financial, childcare, health, and wellness purposes. A more detailing listing of
expenses is available on CNET.
USE OF FUNDS
You can use these funds for expenses incurred during the plan year (July 1 – June 30). You
have 30 days from the end of the plan year to submit for reimbursement..
Reimbursement will only be paid to active, eligible employees for eligible expenses
while an active, eligible employee. Reimbursements will not be paid out to separated or
terminated employees.
Employees must submit a Lifestyle Reimbursement Account Claim Form to the P&A Group
with dated receipts to show proof of purchase. Claims with receipts can be submitted online
at www.padmin.com, via fax at 1-877-855-7105, or via mail. Employees will be reimbursed
through Cary's payroll via direct deposit for approved receipts up to $3,500 less Federal and
State taxes, Social Security, and Medicare.
UNUSED FUNDS
Unspent funds do not roll over to the next plan year and will expire each fiscal year on
June 30.
