MEDICAL
PLAN - TRADITIONAL
To support you and your family, Cary offers two medical insurance options through Blue
Cross and Blue Shield of NC (BCBSNC): Medical — Traditional Plan (PPO) and Medical — High
Deductible Health Plan (HDHP). More information is provided on the next several pages. If
you choose to decline coverage, you must complete the Waiver of Other Coverage form.
MEDICAL - TRADITIONAL PLAN (PPO)
The Medical — Traditional Plan (PPO) includes coverage for routine and wellness office
visits, chiropractic, mental health, substance abuse, and more. Preventive care is covered at
100%, meaning preventive care is FREE.
Most services provided at an office visit are subject to a copay, while all other services are
subject to a deductible and coinsurance, meaning you will be responsible for the cost of the
service until your deductible has been met. Once your deductible has been met, services
will then be covered at 80% coinsurance, until you've reached your out-of-pocket maximum.
Once you've reached the out-of-pocket of $3,000 for an individual and $6,000 for family,
you will not pay for any other medical expenses. The out-of-pocket maximum does not
include pharmacy costs.
If you elect this plan, Cary's $3,500 will be provided through the Lifestyle Spending
Account (LSA) reimbursement program, and you will be eligible to enroll in a
Voluntary Medical Flexible Spending Account (FSA).
CURRENT RATES PER PAYCHECK
If you enroll in the Medical—Traditional Plan (PPO), below are the current rates per paycheck:

You can pay the Reward rate by completing the Healthy Rewards
Program. To
continue earning the reward rate in 2025, employees must complete an annual
physical with a primary care provider AND complete a dental exam with cleaning AND
submit the two forms (physician form and dental form) to WakeMed before December 15,
2024.
If you elect to cover a domestic partner, any amount/rate for the domestic
partner and/or the domestic partner's child(ren) will be paid post-tax and reflected as
such on your paycheck.
