TOWN OF CARY Benefits Handbook Page 10 Medical Insurance MEDICAL – HIGH DEDUCTIBLE HEALTH PLAN (HDHP) OVERVIEW The second option that is available is the Medical – High Deductible Health Plan (HDHP). OVERVIEW The Medical – High Deductible Health Plan (HDHP) covers preventive care services for free. All other services are subject to the deductible and coinsurance. If you enroll in this plan, you will pay the full cost of service until you've reached the deductible and out-of-pocket maximum of $3,200 for an individual and $6,400 for family. Once you've reached the deductible and out-of-pocket, the plan will then pay for covered medical services at 100%, meaning you do not pay for services for the rest of the plan year. The out-of-pocket maximum does include pharmacy costs. When enrolling in the HDHP, you are eligible to enroll in Cary's new Health Savings Account (HSA). Cary will contribute the $3,500 benefit into the HSA. Funds in the HSA can be used for eligible expenses. Cary's contribution is available on July 1 or when the first payroll report is transmitted. Employee contributions are available as deducted per paycheck. If you enroll in the HDHP, you cannot enroll in the Voluntary Medical Flexible Spending Account (FSA). CURRENT RATES PER PAYCHECK If you enroll in the HDHP, below are the current rates per paycheck: STANDARD RATE REWARD RATE Employee Only $67.47 $17.00 Employee + Spouse/Domestic Partner $169.97 $99.36 Employee + Child(ren) $137.01 $64.88 Employee + Family $230.69 $141.75 You can pay the Reward rate by completing the Healthy Rewards Program. If you elect to cover a domestic partner, any amount/rate for the domestic partner and/or the domestic partner's child(ren) will be paid post-tax and reflected as such on your paycheck. HEALTH SAVINGS ACCOUNT When you enroll in the HDHP, you are eligible for an HSA, which is an employee spending account that has tax advantages and investment options, but also spending limitations.
