HELPFUL INFO Benefit Videos Domestic Partner - Enrollment Employee Assistance Program Healthy Reward Program Page 4 CARY Benefits Handbook WHO IS ELIGIBLE FOR BENEFITS All benefit eligible employees who work more than 20 hours per week are eligible to enroll in benefits with Cary. Coverage for all benefits begins on an employee's first day of work. An employee may also cover a spouse, domestic partner, and eligible children. WHEN TO ENROLL IN BENEFITS As a new hire, you have 30 days from your date of hire to enroll in your benefits. After the 30 days, you cannot change your benefits until the annual Open Enrollment period or unless you have a qualifying life event. PROCESS TO ENROLL IN BENEFITS Visit the online benefits enrollment system — Employee Navigator — to enroll in the following benefits: medical, dental, vision, flexible spending accounts, and life insurance. If you do not take any action, you will be enrolled in the Medical - Traditional Plan (PPO) standard rate for employee only coverage. If you decline Cary's medical insurance, you will need to submit the Waiver of Coverage form. You do not need to take any steps to: • Use BHS, Cary's Employee Assistance Program (EAP) • Participate in Wellness Programs, classes, challenges, etc. • Earn paid leave (Leave My Way, vacation, sick, holiday, etc.) PROCESS TO COVER DEPENDENTS You may cover your spouse, domestic partner and/or dependent children up to age 26 on your insurance coverage. Dependent children include: 1. Your biological children, step children or the children of your domestic partner, 2. Your legally adopted child or a legally placed foster child and 3. A child placed with you by an authorized agency or by order of court. Disabled children over the age of 26 may be enrolled on the plan. To enroll dependents, you need their legal name, date of birth, and social security number. Domestic Partners are two adults who are in a committed long-term relationship of indefinite duration with an exclusive mutual commitment and mutual obligation of support for the basic necessities of life, including but not limited to, each other's common welfare and shared financial obligations. Benefits shall include but are not limited to medical, prescription drug, dental and vision insurance and voluntary benefits. For an employee to cover a domestic partner and/or the children of a domestic partner, the employee and domestic partner must complete the Affidavit of Domestic Partnership: Enrollment Form.
