Participant Information Packet For camps that meet 12 hours or more in one week. A participant information packet must be submitted for each camper prior to the start date of the camp. Packets may be found at www.townofcary.org by searching "Summer Camp Forms". If your camper becomes ill or injured during camp, the parent/guardian or emergency contact person(s) will be notified. You should make necessary arrangements to have your child picked up. Be sure to note any medical concerns in the PARTICIPANT INFORMATION–MEDICAL INFORMATION portion of the packet. Please fill out Section 3 of this page if medication needs to be administered during camp hours (including asthma medication). This section must be completed in its entirety and signed by parent/guardian before any medication will be dispensed. Medication will be secured in a locked box at all times. Camp counselors will carry medication to dispense as needed while on field trips. Dress Code Campers should dress accordingly for the activities scheduled. It is recommended they wear shorts, a light weight shirt, and some type of athletic shoe or sneaker in order to participate in recreational and athletic activities each day. For programs at the Cary Arts Center, it is recommended campers wear old clothing. Certain camps may have additional clothing requirements. Sandals, flip- flops, rubber sandals, and other open toed shoes are not recommended as acceptable attire for active recreation activities. If the camper is not properly attired, the parent/ guardian will be notified to bring appropriate clothing or will be required to pick up the camper. Personal Belongings The Town of Cary and staff are not responsible for camper's personal belongings. Campers should not bring toys, cell phones, electronic devices, jewelry, or money (except for concessions). Put camper's name on all articles of clothing, towels, lunchboxes, and bags. Sunscreen and Insect Repellent It is recommended that campers apply sunscreen and/ or bug spray prior to arriving at camp. Camper may bring sunscreen/bug spray, labeled with camper's name, to camp; however, they must apply it themselves under adult supervision. These items may not be shared with others. Behavior Management Camp staff strive to create a fun and safe environment for all participants. Praise and positive reinforcement are used as effective methods of behavior management. Camper behavior that does not respond to these methods or is destructive to others or property will be dealt with in a professional, positive, and timely manner to correct the behavior. The following procedures will be followed for behavior management. All Incident Reports will be discussed privately with parents/guardians and a copy of each report will be kept on file in the camp office. 1. In the event that a camper's behavior is a repeated behavior and cannot be corrected by Camp Staff with a verbal warning or other form of behavior modification, a first Incident Report will be written to document and correct behavior. A copy of the report will be given to the parent/guardian the same day.
